How To Get Started

If this is your first time with us or you are returning, please schedule an Intake Appointment from the top of the Client Portal.

See below what it should look like.

Be sure you are selecting the correct Appointment Request.

-Request for Existing Patient

Or

-Request for New Patient

Once you schedule an Intake Appointment, there will be forms for you to complete. Bring any and all documents you may need. Please call us if you have questions 🙂
